Welcome to CityHealth’s In-Home COVID-19 Test!
Please follow the instructions below to activate your kit and collect your sample.

Activate your kit in 3 easy steps
- Enter the Registration Code from the card in your test kit
- If you are a new CityHealth Patient, fill out your Patient Profile
- Proceed to your Virtual Waiting Room, where you will video chat with a provider who will show you how to collect your sample.
Have this info handy:
Important: COVID samples must be shipped on the same day they are collected.
Please do not collect or ship your sample on Saturday or Sunday, as the sample will not arrive at the lab on time.
Do not remove the barcode label from the collection tube.
If your collection tube does not have a barcode label, please contact customer service using the live chat.
Start by locating the registration card in your test kit.
Then, enter your unique registration number from the card into the box on the right and click Next Step.
