At-Home Test Kit

Locate your registration card for your registration code.

Government Issued ID

For children, parents or legal guardians will be required to upload a valid government-issued ID.

Health Insurance Card

You may be asked to upload a photo of your insurance card.

Enter the unique code from your registration card in your test kit. Do not remove the barcode label from the collection tube.

To complete your patient profile, have your Government Issued ID and Health Insurance information ready and handy!

Terms & Conditions

Carefully read and agree to all terms and conditions. COVID samples must be shipped on the same day they are collected.

Virtual Waiting Room

Proceed to your Virtual Waiting Room, where you will video chat with a nurse who will show you how to collect your sample.
