It is very important that you get tested by a Certified Trusted Testing & Travel Partner such as CityHealth an affiliate of HavenMD. Test from a non-certified partner will not allow you to bypass the mandatory Hawaii state quarantine.
The State of Hawaii’s pre-travel testing program requires passengers five years and older to take a Nucleic Acid Amplification Test (NAAT) from a Clinical Laboratory Improvement Amendments (CLIA) certified laboratory within 72 hours from the final leg of departure. CityHealth offers three types of tests that meet this specification and fulfill the State of Hawaii’s requirement.

CityHealth can test infants as young as 3 months, however, the state of Hawaii only requires tests for those 5 years and older. As part of the intake process, have your upcoming Four Seasons reservation confirmation ready to present to a CityHealth clinician. You’ll then receive a COVID-19 test. Please see our pricing and testing options below to determine the test best suited for your situation. Results will be ready within 24 – 72 hours depending on the test.
Get your results & you’re off
Your test results will be available via OnPatient.com. You will receive an email message when your results are ready, and you’ll be able to log into the OnPatient portal to obtain the result of your test. You must save the document as a PDF. It is very important that you sign up for Hawaii Safe Travel website 24 hours prior to your departure and upload your PDF .
Available Testing Options
This is considered the “gold standard” test with the highest degree of reliability. The test takes a few moments and the sample is sent overnight to a lab that processes the test and the result. Results are transmitted by e-mail typically in 24 to 48 hours. It is important to note that the Hawaii Safe Travels program “72-hour clock” starts at the time the sample is collected. This test option is available at all of our testing facilities.
The Abbott ID NOW COVID-19 Test is the fastest available molecular point-of-care test for the detection of novel coronavirus (COVID-19). Developed by Abbott, it has been authorized by the FDA under an emergency use authorization for use by authorized laboratories and patient care settings. The test has been authorized only for the detection of nucleic acid from SARS-CoV-2, not for any other viruses or pathogens, and is only authorized for the duration of the declaration that circumstances exist justifying the authorization of emergency use of in vitro diagnostic tests for detection and/or diagnosis of COVID-19 under Section 564(b)(1) of the Act, 21 U.S.C. § 360bbb-3(b)(1) unless the authorization is terminated or revoked sooner. This test is only available through "Concierge Testing". The cost for this test is $750 for two travelers, $250/additional traveler.

Q&A Before Visiting Hawaii
All adults (18 years and older) must register on the state of Hawaii Safe Travels digital system. You will be required to upload your negative pre-travel test result and answer a health questionnaire 24 hours prior to your departure.
Each child traveling with an adult should be included in the adult’s Safe Travels’ form.
Beginning October 15, passengers who successfully complete a pre-travel test will not be subject to the state’s 14-day quarantine program. FDA-approved NAAT tests, processed by a CLIA-certified laboratory are the only types of coronavirus tests currently approved by the state of Hawaii. The state of Hawaii will ONLY accept test results from TRUSTED TESTING AND TRAVEL PARTNERS: AFC Urgent Care, Bartell Drugs, Carbon Health, CityHealth Urgent Care, Color, CVS Health, Discovery Health MD, Kaiser Permanente, Quest Diagnostics, Vault Health and Walgreens (as of 10/13/20).
Yes, minors five years and older must take the pre-travel test. (Minors four years and younger do NOT need to take the pre-travel test.)

Within 72 hours from the final leg of departure. If you have a multi-city itinerary, it’s the departure time at the last city before arriving at a Hawaii airport.
A traveler who takes a test within 72 hours from the scheduled final leg of departure, and has a negative test result, and whose departure is delayed by the airline, will not be subject to the state 14-day quarantine. This provision will not apply to any additional county-imposed quarantine rules separate and apart from the state's pre-test waiver.
Upload your negative result** on your Safe Travels account. 24 hours prior to boarding your flight, log on your Safe Travels account and answer a health questionnaire and receive a QR code.
**Please keep this negative test result with you as you may be asked to show your negative test result upon checking in at your hotel or motel accommodations.
The state has a multi-layered screening process to protect the health and safety of everyone traveling to the Hawaiian Islands. All incoming travelers will have their temperature taken by thermal screen.
