You will need to provide identification at the test site prior to the appointment.

The appointments booked through this link will receive the Moderna vaccine, which requires 2 doses for full efficacy. By choosing an appointment, you will also automatically be enrolled to a second dose appointment four weeks from the original. You will have an opportunity to modify your second appointment if needed after confirming your first appointment, by the confirmation email you will receive for both appointments.
